Taking care at home
Our Ambulatory Care Manager program helps ensure patients keep follow-up appointments; have needed prescriptions and information; and receive referrals to community resources as necessary. These interventions help patients avoid coming back to the hospital or the emergency department, resulting in better outcomes. The program is especially beneficial to our palliative care patients and their families.
